With the increased use of dermal fillers, especially HA, came early and late complications with varying degrees of severity. The incidence of complications is low and most adverse events are mild (edema, erythema and local ecchymosis) and of short duration. However, more serious events such as ischemia and necrosis can occur. This article aimed to report a clinical case of lower labial tissue necrosis associated with the injection of HA as well as the treatment performed. As a methodological foundation, a bibliographical review of a qualitative approach was presented, with the support of a clinical case, which was based on the literature review of the last ten years. It was concluded that the first strategy in the face of vascular complications associated with the injection of dermal fillers is prevention, and the professional injector must have knowledge of the vascular anatomy of the topography as well as the characteristics and indications for use of the filler material. Professional injectors should be aware of the signs and symptoms related to complications and be prepared to treat them with excellence and confidence, and, although there are protocols for the treatment of vascular intercurrences associated with the use of fillers based on HA, there are not yet a specific ideal treatment for each situation, making further studies in this area necessary, in view of the significant increase in the use of fillers in recent years.
