It is important for the dentist to recognize and act on this type of common lesion on the mucosa. This report aimed to describe and discuss the clinical management, diagnosis, treatment, and followup of a peripheral giant cell granuloma in an immunosuppressed patient. Male patient, 61 years old, previously received a hepatic transplant and used immunosuppressive medication. During dental treatment, he complained of the appearance of a lesion in the alveolar ridge of the rounded mandible, exophytic, asymptomatic, red-purple color, pediculated, measuring 1 cm in diameter without ulcerations on the surface and without radigraphic evidence of bone involvement. With the diagnostic hypothesis of PGCG, complete removal of the lesion was carried out and the histopathological examination confirmed the clinical diagnosis. The case reported shown the importance of the correct professional management of the patient, promoting a premature and humanized diagnosis.
