The dental elements that most frequently present total or partial inclusion are the lower third molars, the third upper molars, the supernumerary teeth, and the superior canines. The elderly patient presents surgical needs that increase in complexity, because he presents a most compact alveolar bone and with reduced capacity of dilation, besides being able to present systemic diseases, nutritional deficiencies, and other conditions associated with the aging process that make him a special case. The aim of this research is to report the case of a female patient, 86 years old, with an included/impacted and symptomatic (pain) upper left canine, thus being an absolute indication for exodontia, and discuss care in the exodontia planning and execution of the included element, evaluating the systemic conditions of the patient, who presented high blood pressure, cardiac arrhythmia with implantation of a cardiac pacemaker, generalized arthrosis, and used different medications continuously. Dental surgeons need, according to the possibilities of each patient, to know the best time for the surgical approach of an included tooth. In the case reported in this study, the patient’s advanced age and systemic complications represented a factor of greater attention, as the exodontia of the element in question was not complicated due to the absence of all the dental elements of the region and the physiological bone resorption presented.
