The loss of keratinized gums and interdental papillae can cause problems such as impaction of food and peri-implant inflammation, especially in the posterior region. This case report aims to demonstrate, through a clinical case report, how regeneration procedures associated to a specific prosthetic connection can prevent the collapse of peri-implant structures and ensure tissue health and stability from the extraction, immediate implantation and rehabilitation of a compromised posterior dental element. It was concluded that the immediate implant placement is effective for the maintenance of the alveolar crest and that the use of biomaterials can minimize bone resorption and improve the stability of peri-implant tissues.
