This work was made to examine the cephalometric alterations resulting from the extra-oral force applied to the upper molars with cervical anchorage. A literature review was accomplished, encompassing the mechanics of these appliances. The behaviour of the maxilla and mandible was evaluated individually and between each other. Also the alterations in the growth standard and in the position of the first upper molars in relation to the therapy with an extra-oral appliance supported by upper molars were appraised. Cervical anchorage can be recommended for the correction of the sagital discrepancy between the maxilla and the mandible, in Class II cases, to individuals with predominantly horizontal growth pattern, and should be avoided to individuals with predominantly vertical growth pattern.
