Orthodontic approach of ankylosis resulting from dental trauma – case report

Vol. 12 – Number 48 – 2019 Case report Page 40-52 Orthodontic approach of ankylosis resulting from dental trauma – case report Fabiana Soares Wilken1 Rodrigo Hermont Cançado2 Leniana Santos Neves2 Bruno Leonardo Rocha3 Alexandre Fortes Drummond4   Abstract Traumatic dental injuries occur frequently in Dentistry, and it is the dental surgeon’s duty to know how to diagnose and treat them accurately and quickly. One of the rarest and most difficult to treat lesions is the intrusive luxation, in which the traumatized element is taken to the axial position. For treatment, in most cases it is recommended to wait for the passive reerection of the same, and, in case of failure, to perform the orthodontic or surgical traction. One of the possible consequences of the intrusive luxation is dental ankylosis, characterized by fusion of the root to the bone, generating immobility of the traumatized element. There is no way to stop or reverse the ankylosis process. Therefore, it is necessary to perform some interception, there are several types of treatments for ankylosed teeth. Treatment plans should be planned and discussed together with the patient, explaining their advantages and disadvantages until the most favorable prognosis is achieved, including the patient’s needs and desire. The article reports a clinical case of a permanent maxillary central incisor that suffered intrusive luxation and that after orthodontic traction performed with failure, resulted in dental ankylosis. For treatment of choice the extraction of the ankylosed element and repositioning of the adjacent elements was carried out, followed by dental  reanatomization. The pros and cons of this type of treatment will be discussed.
