There are several protocols for its correction such as: extractions, use of elastics and functional appliances. Some factors have to be considered during the diagnosis in order to reach an assertive treatment plan and not to bring harm to the patients. Premolar extractions is an effective and efficient way, but it can bring undesirable alterations to the facial profile. In this context orthodontists can make use of more conservative options. When elastics are used, the treatment success depends largely on patient compliance. If the patient is non-compliant, the sagittal correction and the treatment as a whole end up being compromised. On the other hand, fixed functional appliances are a favorable option to the Class II correction, because it does not depend on patient compliance and are relatively comfortable. The aim of this article is to present a clinical case of a bilateral Class II with mandibular retrusion treated with the fixed functional appliance Forsus. The treatment was performed without extractions in order to avoid the retraction of the superior lip and increase the protrusion of the inferior lip, improving the patient’s profile and consequently her self-esteem. This appliance proved to be efficient promoting predominantly higher dentoalveolar changes. Being a relatively comfortable appliance, it was well accepted by the patient who cooperated and was adept to this treatment modality. It was concluded that the fixed functional appliance Forsus is effective to correct Class II.
