Use of botulinum toxin in the control of bruxism – case report

Vol. 4 – Number 14 – 2023 Case report Page 46-52 Use of botulinum toxin in the control of bruxism – case report Yara Janaina Viana Lima Lido¹ Alexandre Miyahira² Pedro Carvalho Feitosa³ ABSTRACT Bruxism is a parafunctional masticatory activity characterized by excessive clenching and/or grinding of teeth, which may occur during sleep or wakefulness. Several causes have been researched, including psychological factors, malocclusion, sleep disorders, and some medications. This disorder affects from 5 to 96% of the population, to some degree, during some period of life, bringing various symptoms such as: headache and muscle pain, tooth attrition and fracture, and temporomandibular joint dysfunctions. Currently, botulinum toxin type A is used for various medical and cosmetic conditions. In recent years, there has been an increase in the use of botulinum toxin to control bruxism, being applied to the masseter and temporal muscles. There are reports of an average reduction of 20% in masticatory force and contraction of the involved muscle over the course of 6 months. The present case report study aims to analyze the effect of botulinum toxin on a patient diagnosed with bruxism and masseteric hypertrophy. The evaluation will be done through photographs before and after the procedure and a questionnaire to be answered by the patient. 30U of botulinum toxin will be used in each masseter, divided by 3 injection points, and in the anterior portion of the temporal muscle, 20U on each side. After three months of application, a new evaluation and photographs will be carried out, and it is expected that there will be a decrease in the events of clenching and grinding of teeth, a decrease in pain and muscle hyperactivity, and a reduction in the volume of the masseter.
