The patient is considered skeletal Class III when manipulated in centric relation (RC) and centric occlusion (OC) remains with the repositioning of the mandible to anterior. This article reports a clinical case of anterior crossbite of an adult patient, skeletal Class III undergoing compensatory treatment. The objective was to correct anterior crossbite and the occlusal plane, establishing an adequate intermaxillary relationship. The anterior crossbite was resolved, a bilateral Class I canine was obtained, the present crowding was eliminated, the occlusal plane was leveled. Compensatory treatment of a Class III skeletal malocclusion is more accepted among patients, however it is a challenge for orthodontists. The present study presented a possibility of compensatory orthodontic treatment in an elderly Class III patient, who obtained a satisfactory aesthetic and functional result, in order to provide some guidance for cases of a similar peculiarity. The projection of the upper incisors at the beginning of the treatment was crucial for the camouflage of the skeletal pattern, eliminating the crossbite and obtaining a significant response from the upper lip.
