Paulo César Simamoto Júnior1 Morgana Guilherme de Castro 2 Veridiana Resende Novais3 Maiolino Thomaz Fonseca Oliveira4 Clébio Domingues da Silveira Júnior 5 Resumo Neste caso clínico o paciente utilizava um protocolo inferior sobre três implantes quando um dos implantes foi perdido. Diante disso, foi planejada a reposição do implante perdido e a readequação de sua prótese por meio de solda TIG (TungstenInertGas). Inicialmente, os modelos superior e inferior foram montados em articulador semi-ajustável com a própria prótese do paciente e posterior confecção de registro oclusal de silicone. Os dentes da região do implante perdido foram retirados bem como a resina acrílica que os fixava. Numa etapa clínica, o implante perdido foi removido e imediatamente substituído por outro. Em seguida a prótese foi seccionada e as partes posicionadas na boca do paciente sobre seus respectivos implantes. Após união dos dois fragmentos em boca a estrutura foi reposicionada no modelo. No laboratório, um novo análogo foi colocado na região do implante perdido e novo gesso foi vazado nessa região para fixá-lo, em seguida foi realizada a soldagem dos segmentos com a solda TIG. Procedeu-se então o reposicionamento dos dentes sobre a barra, com o auxílio do registro de silicone, e o espaço restante foi preenchido com resina acrílica da mesma cor que o restante da prótese. Conclui-se que a utilização da solda TIG representou uma técnica eficiente e de relativo baixo custo, se comparada à solda a laser, para soldar infraestrutura já recoberta com material estético. Para o paciente o ganho se reflete também na possibilidade de ter seu implante substituído e sua prótese reinstalada no período de apenas um dia. Descritores: Soldagem em odontologia, implantes dentários e titânio Abstract The present study reports the case of a patient who had a mandibular fixed prosthesis supported by three implants, when one of the implants was lost. Therefore, were planned the reposition and adjustment of the implant using Tungsten Inert Gas (TIG) welding. Initially the mandibular and maxillary casts were placed on a semi-adjustable articulator with the patient’s prosthesis and posteriorly an interocclusal register was made in silicone. The teeth from the region of the lost implant, as well as the acrylic resin used to grip them, were removed. In a clinical stage the lost implant was extracted and immediately substituted for a new one. Then the prosthesis was sectioned and the parts placed over its respective implants. After both fragments were united in the patient’s mouth the structure was repositioned on cast. At the lab, a new analog was placed in the region of the lost implant and the cast was repaired to fix it and then it was performed the TIG welding. Teeth were repositioned over the bar, with the help of a plastic record and the remaining space was filled with acrylic resin in the same color of the rest of the prosthesis. It was concluded that the TIG welding is an efficient technique to weld structure already covered by aesthetic material, and relatively inexpensive when compared to laser welding. For the patient the benefit is the possibility of having its implant replaced and its prosthesis reinstalled in the same day.
