Entretanto, os estudos clínicos têm focado nos parâmetros que influenciam a interação entre o implante e seus tecidos adjacentes. Este trabalho relata o caso clínico de um paciente do gênero masculino que procurou atendimento odontológico por motivo de fratura radicular do elemento 22 provocado pelo núcleo metálico fundido. Foi planejado exodontia atraumática e imediata instalação de implante cone morse à 4 mm sub-ósseo em virtude da ausência da tábua óssea vestibular, considerando apenas a tábua palatina. Não foi realizado enxerto ósseo ou gengival por se acreditar que este defeito ósseo de quatro paredes seria reparado apenas pela formação de coágulo sobre as roscas do implante. Em seguida, foi selecionado o munhão, realizou-se sutura e posterior confecção de provisório imediato. Foram feitos ajustes oclusais retirando-se os contatos cêntricos e excêntricos. Após o período de 6 meses, o paciente retornou ao consultório para reabilitar toda bateria anterior. Nessa oportunidade, foi solicitada tomografia computadorizada da região. Os cortes tomográficos mostraram que houve formação óssea na região do defeito vestibular, mesmo sem ter sido realizado procedimento de enxerto. Este fato pode ser explicado por fatores como tratamento de superfície do implante utilizado, conexão protética do tipo cone morse e biotipo gengival. Clinicamente, notou-se um abaulamento na vestibular do rebordo na região do implante 22, porém sem nenhuma repercussão estética considerável. Abstract Fixed prosthesis over implants for the rehabilitation of a single element in the anterior maxilla is a fairly predictable treatment modality and with a high success rate. However, clinical studies have focused on parameters that influence the interaction between implant and its adjacent tissues. This paper reports a clinical case of a male patient who sought for dental care because of a root fracture of 22 element caused by molten metal core. It was planned an atraumatic extraction and an immediate implant installation of a morse taper 4 mm sub-bone due to the absence of buccal bone considering only the palatal bone. Bone or gingival graft weren´t performed because it is believed that this bone defect with four walls would be repaired only by clot formation on the threads of the implant. After that, the abutment was selected,  insuture was realized and the immediate provisional restoration was made. Occlusal adjustments were made, by removing the centric and eccentric contacts. After a 6 month period, the patient returned to the clinic to rehabilitate all the anterior battery. At that time a computerized tomography of this region was solicitated. The tomographic slices showed that there was bone formation in the buccal defect region even without being held grafting procedures. This fact can be explained by factors such as surface treatment of implant used, prosthetic connection type morse taper and gingival biotype. Clinically, it was noticed a bulge in the buccal region on the edge of the implant 22, but without any significant aesthetic impact.
